which I intended to be Registered in The Office of the Administrator and Registrar- General, Freetown in the Western Area of the Republic of Sierra Leone. I undersigned VERONICA KUMBA BANGALIE of Block 4b Bo Correctional Barracks, Bo, Kakua Chiefdom, Bo District in the Southern Province of the Republic of Sierra Leone, now lately Called VERONICA KUMBA MARGAI a citizen of the Republic of Sierra Leone by Birth and an Adult DO HEREBY:-

1.For and Behalf of myself wholly renounce, relinquish and abandon the use of my former name of VERONICA KUMBA BANGALIE in place place thereof do assume from the date hereof the name of VERONICA KUMBA MARGAI and so I may hereafter be called known and distinguished not by my former name of VERONICA KUMBA BANGALIE but by my assumed name of VERONICA KUMBA MARGAI.

2. For the Purpose of Evidencing such my determination declare that I shall at all times hereafter in all records, deeds and writing and in all proceedings, dealings and transactions as well as private as public and upon all accasions whatsoever use and sign the name of VERONICA KUMBA MARGAI as my name in place of and in substitution for my former name of VERONICA KUMBA BANGALIE.

3. Expressly authorize and request all persons at all times thereafter to designate and address me by such assumed name of VERONICA KUMBA MARGAI accordingly.
